The AAO has designated ITS as the official registration and housing company. Questions may be directed to 800.424.5249 (U.S. and Canada) or 847.940.2155 (International) or by e-mail to aaortho@itsmeetings.com.
You must register for the meeting at the same time as or prior to reserving housing in Palm Springs. The registration and housing form is located at the back of this booklet. You can also register online. Registering online is the fastest way to secure your registration confirmation for this limited attendance conference.
Registrations received after Wednesday, January 3, 2007, will be subject to the late registration fees listed below. Mailed forms should be sent in as early as possible to avoid mailing delays.
Note: If you register after January 3, 2007, you must book your housing directly with the hotel. Housing is subject to availability and a possible rate increase.
Spouses/guests may purchase tickets on-site for $390 (U.S.) to attend the Exhibit Hall and food functions only. (Note: The $390 will be accepted on-site only. Please do not add $390 to your registration form that is located in the back of this book or online.) This ticket purchase does not allow access to lectures or continuing education.
Registration and housing will be confirmed by e-mail or fax within seven business days. You cannot register or reserve housing by telephone.

ITS will mail your name badge, name badge holder and pertinent on-site information about three weeks prior to the meeting. Please bring these materials to Palm Springs.

Housing Information
The Renaissance Esmeralda Resort & Spa is the location of this conference. Please review your hotel confirmation carefully to ensure the arrival and departure dates are correct. If you register after January 3, 2007, you must book your housing directly with the hotel. If you book your housing after January 3, 2007, you may be subject to a higher room rate. Housing, which is subject to demand and space availability, may close prior to January 3, 2007.
A deposit of $312.00 (U.S.) for each standard guestroom reservation will be required. (The $312.00 room deposit charge includes tax.) All reservations must include the deposit; please provide a valid credit card number or a check made payable to “AAO/ITS.” Your credit card will be billed immediately for the deposit.
Room rates are $285 (U.S.) single/ double (plus additional local tax) per night. There is a one-time porterage fee of $10 per person and a nightly housekeeper fee of $2. Both fees and local tax are in addition to the room rate of $285 and will be added to your individual guestroom charges.

Videotaping Policy
Attendees and exhibitors are not allowed to audiotape or videotape any portion of this meeting. This includes all lectures, other meetings, the Exhibit Hall, any Exhibit Hall activities, or any other portion of this conference.

Continuing Education Credit Hours (CE Hours)
The CE Hour Reporting System does not require you to scan a card outside the individual session room, and it also does not require session codes. Computers will be available outside the Crystal Ballroom. Just follow the instructions on the screen to select attended sessions and to print your CE report.
Or, if you prefer to avoid lines and waiting, reports may be printed from the convenience of your home or office computer following the conference. Go to the AAO Web site, www.AAOmembers.org, and click on the link for the AAO CE Credit Manager.
The AAO is an American Dental Association (ADA) CERP Recognized Provider.
Please note that due to varying state/province/country continuingeducation requirements, it is the responsibility of the individual attendee to print their own CE Hours Report and to submit any necessary continuing-education credit documentation to their individual state/province/country on course verification. Please contact your state/province/country directly to determine what items are necessary to satisfy your continuing education requirements. The AAO will NOT mail CE Hours Reports.

The AAO will videotape a portion of this conference. The taped lectures will be available for purchase on CD-ROM or online at a later date. The Exhibit Hall will be located in the Emerald Ballroom and will contain the latest in orthodontic specialty products and services available to the practicing doctor. Lunch, beverage breaks, and cocktails-only receptions will be served in the Exhibit Hall for your convenience.
Spouses/guests may obtain a name badge for the Exhibit Hall and food functions only by purchasing a ticket for $390 (U.S.). This ticket purchase will only be available on-site and does not allow access to lectures or continuing education.

Scientific Program Policies
Cellular telephones, pagers and other electronic devices must be turned off or turned to silent mode. Please be courteous to the other attendees and leave the room if you need to converse.
The lectures are restricted to registered doctors only. Spouses/ guests and children are not allowed in the lecture hall at any time.
